Want to join a CAP Sports team? Sign up in the forms below.
- Register and pay the athletic fee online through FamilyID
- Family ID: https://students.arbitersports.com/organizations/community-academy
- Athletic Fee: $50
- Submit a completed PIAA physical (must be filled out by a healthcare provider and turned in to the Athletic Department). If you played a fall or winter sport you are not required to submit again.

- Complete Impact Concussion Testing, if required
- Testing will be administered by Mr. Bowers
- Students who participated in a fall or winter sport do NOT need to resubmit a PIAA physical or complete Impact Testing again

Failure to complete these requirements by the deadline may affect participation on the team.
