Physical Forms & Information

  • Sections 1-5 to be completed by parent/guardian
  • Section 6 to be completed by a primary care physician
  • If participating in any additional sports in the same school year, complete Section 7 of the form and submit it to the Athletic Director. We will then decide if Section 8 is required.
  • Secciones 1-5 deben ser completadas por el padre/tutor.
