The Health Office would like to share a few important reminders and updates to keep our students healthy and ready to learn.
Physical Examination Reminder: If you have not sent in a completed physical examination form for the school year 2026-2027, please submit it as soon as possible. Physicals are required for all grade levels.
Dental Examination Reminder: Dental health is an important part of a child’s overall well-being. If you have not sent in a completed Dental examination form for the school year 2026-2027, please submit it as soon as possible.
Medication at school: If you child needs medication during the school day, the following must be provided:

If your child will be entering 7th or 12th grade next year, please remember that all mandated immunizations must be submitted before the new school year begins. These forms can be found further down in the newsletter.
These documents are important to help ensure your child remains in compliance with school health requirements and can participate fully in school activities.

Important Reminders: - Please submit all or any updated immunizations to the health office as soon as possible. You may email those to [email protected] - Please submit updated Physicals and Dentals - Please notify the health office of any new medications, changes in health status, updated action plans for asthma, seizures, or chronic conditions. Medications must all be in original container or box with your child’s full name. Medications required to be administered in school must have a form submitted completed and signed by the physician.

Health Services are available to all CAP students and have been established to be a liaison between home and school regarding health concerns. The nurse's office is open every day for medication administration, First Aid, and sick childcare.
The following chart may assist you in keeping track of health screenings.
The Pennsylvania School Health Code requires that the following immunizations be completed within 5 days of the start of school for a student to attend school:
- DTaP (diphtheria, tetanus, and acellular pertussis) - 4 doses, one given after the 4th birthday;
- Polio - 4 doses, one given after the 5th birthday and at least 6 months after previous dose;
- MMR (measles, mumps, and rubella) - 2 doses after 12 months of age;
- Hepatitis B - 3 properly spaced doses, with the last dose given after 24 weeks;
- Varicella (chicken pox) - 2 doses after 12 months of age or written proof of the disease from your health care provider
- Tdap (tetanus, diphtheria, and acellular pertussis) - on the 1st day of 7th grade;
- Meningococcal Conjugate - 7th grade requirements
- Meningococcal Conjugate - 2nd dose if first dose given before age 16 - 12th grade requirements.
