Diploma Requests Form

All Diploma requests must be submitted to the Office of Admissions and Student Records along with the appropriate fees.

Requests must include the following information:

 

  • Complete Name: (Please be sure to give us the name you used during your enrollment at CAP/Phila. Community Academy/The Community High School/Community Voyage)

  • Date of Birth

  • Approximate dates of attendance

  • Graduation Year

  • Complete current address

  • Complete the name and address where you want your diploma sent

  • Diploma Request Forms can be submitted by fax, mail, email, online, or drop-off

  • Fax number 267-345-0530

  • Mailing address: 1100 East Erie Avenue, Philadelphia, PA 19124

  • Email address: [email protected]

  • You can also drop it off at the school. Please put it in a sealed envelope and title Attn: Student Services                                 

                                     Diploma Request Fee is $5.00 per copy (Money Order or Cash ONLY)

2. Step 2

Diploma Request Forms can be submitted by fax, mail, email, or drop off.
