Transcript and Diploma Requests

All transcript/diploma requests must be submitted to the Office of Admissions and Student Records along with the appropriate fees.

Requests must include the following information:

  1. Complete Name: (Please be certain to give us the name you used during your enrollment at CAP/PCA/CHS.

  2. Date of Birth

  3. Approximate dates of attendance

  4. Graduation Year

  5. Complete current address

  6. Complete name and address where you would like to have your transcript/diploma sent.

Please note (ONLY FOR TRANSCRIPTS) that these fees do not apply to current students or graduates of the years  2021,2020,2019,2018,2017

Step to Request your Transcript/Diploma

Transcript/Diploma Request Forms can be submitted by fax, mail, email, or drop off.

  • You can also drop it off at the school. Please put in a sealed envelope and title Attn: Admissions office

***We MUST receive the completed request form AND payment in order to process the request.***

Transcript Fees:

$5 per transcript

 

Diploma Fees:

$5 per copy of diploma

Original diploma: Price will be determined upon receipt of the diploma from Jostens.
